Hubert Zajac Bio

Hubert Zajac, DPT, is a licensed physical therapist serving patients in Hermitage and Lebanon, Tennessee, with expertise in vestibular therapy, dizziness and vertigo treatment, TMJ disorders, and orthopedic rehabilitation. He provides personalized, evidence-based care to help patients improve mobility, reduce pain, and restore function.


Originally from Poland, Hubert moved to Chicago, Illinois as a teenager. He earned his Bachelor of Science in Biology from Loyola University of Chicago, followed by a Master of Science in Integrative Physiology from Benedictine University in Lisle, Illinois. He then completed his Doctor of Physical Therapy degree at Rosalind Franklin University of Medicine and Science in Chicago, Illinois.


During his doctoral training, Hubert served as the manager of a pro bono physical therapy clinic, providing care to underserved communities—an experience that reflects his commitment to accessible, compassionate healthcare.


Hubert is a licensed physical therapist through the National Physical Therapy Examination (NPTE) and brings experience from both outpatient and inpatient rehabilitation settings, allowing him to treat a wide range of conditions.

In 2025, Hubert and his wife relocated to Nashville, TN, as she pursues a career in Anesthesiology at Vanderbilt University Medical Center. Outside of his professional life, he enjoys staying active through soccer, hiking, pickleball, and cheering on his favorite team, FC Barcelona.
